Getting Started - Intake Process, Forms, Medications

Call, Text, or use the self-scheduling link to request an appointment. Your appointments may be either virtual or in person. Please comment to request either an in-person or video call. Please allow 48 business hours when scheduling an appointment to be seen to ensure enough time for the required paperwork and releases to be completed, returned and reviewed by our providers prior to your appointment time.

Once your scheduled appointment has been accepted, you will receive an email with the necessary intake forms and releases that need to be returned, as well as an invite you to activate and access your personal patient portal online.

From the patient portal, please view your appointment and complete the initial intake paperwork. This paperwork, a copy of your drivers license, as well as a copy of your insurance card (front and back) must be returned no later than 48 hours prior to your appointment. This allows our billing department to verify insurance etc.

Please make your payment, including co-pay, through the website below. If you have questions regarding this process, please contact our office via phone or text.

Your provider may ask you to complete other forms that are available for you to download below.

PRESCRIPTION POLICY: Prescription refills may take 1-3 days to process and complete. Please allow yourself at least three days to get the medications filled. Please plan ahead if the prescription is due on a weekend or holiday, and give us enough time to prepare the prescription.
