Appointment Policy:  WE TAKE IN-PERSON AND VIRTUAL APPOINTMENTS (when self-scheduling, please read the important notes located under the "available times" section before selecting your appointment in order to include needed information and time needed to accommodate requests)

Your time is valuable and so is ours. When making appointments we reserve time for you and we plan around that reserved time out of respect for your time.  When you do not show up for your appointment, you are taking away valuable time that could have been used for someone else who needed to be seen. We therefore require self-pay patients to pay a $50 deposit at time of appointment confirmation and will be applied to the patient's account. At time of appointment, remaining charges are collected prior to seeing the provider. If we are to bill insurance, we need your insurance info right away in order to determine what cost you will be responsible for. If you have a co-pay, need to meet a deductible, or a % cost share, that amount will need to be paid prior to seeing the provider. Any appointments not kept (no show or “late”) or rescheduled 24 hours prior to a Tuesday - Friday appointment or 48 hours for a Saturday - Monday appointment, will forfeit their deposit, or be charged a $50 no show/late notice fee. If you are “late” to your appointment you may not be seen if there is not enough time allotted required by the insurance company to be billed. Self-pay may be seen “late” for remaining minutes if paid in full if the patient and provider agree. (See Full Financial Policies with your intake forms for more information).

Provider Cash Pay Charges (non-insured):

• Initial, new patient evaluation and diagnostic 45-60 mins . . .  . $250.00 - $375.00   
• Follow up appointments for medication management/psychotherapy up to 30 mins .  . . .$125.00 - $250.00
• Telephonic quick patient contact [items that can't wait until regular appointment time] up to 10 mins .. $50  
• Some cash paid programs with closely monitored prescriptions may require multiple visits per month. Therefore a special rate for needed subsequent follow-up visits in the same month will be provided. 
• We also may provide special pricing throughout the year for services such as sports physicals and will be charged ‘cash fee for service’ as advertised. 
• Also ask about our cash payment plan option - 1 year plan - which included your initial evaluation and 4 additional check-ins- timely scheduled through out the calendar year with prescription refills for $100 up front and then $60 a month. If you wish to continue payments after the first year it is $42 a month with quarterly visits.
