Appointment Policy: WE TAKE IN-PERSON AND VIRTUAL APPOINTMENTS (when self-scheduling, please read the important notes located under the "available times" section before selecting your appointment in order to include needed information and time needed to accommodate requests)
Your time is valuable and so is ours. When making appointments we reserve time for you and we plan around that reserved time out of respect for your time. When you do not show up for your appointment, you are taking away valuable time that could have been used for someone else who needed to be seen. We therefore require self-pay patients to pay a $50 deposit at time of appointment confirmation and will be applied to the patient's account. At time of appointment, remaining charges are collected prior to seeing the provider. If we are to bill insurance, we need your insurance info right away in order to determine what cost you will be responsible for. If you have a co-pay, need to meet a deductible, or a % cost share, that amount will need to be paid prior to seeing the provider. Any appointments not kept (no show or “late”) or rescheduled 24 hours prior to a Tuesday - Friday appointment or 48 hours for a Saturday - Monday appointment, will forfeit their deposit, or be charged a $50 no show/late notice fee. If you are “late” to your appointment you may not be seen if there is not enough time allotted required by the insurance company to be billed. Self-pay may be seen “late” for remaining minutes if paid in full if the patient and provider agree. (See Full Financial Policies with your intake forms for more information).
